Statement of Understanding for the At-Home Lab Activities
Mt. San Antonio College requires that all students receiving an “At-Home Laboratory Kit” read and sign the following statement of understanding.
Please read the entire statement carefully.
Although the contents of the kit and supplies are generally safe, the equipment can be hazardous if used incorrectly.
- I understand that by participating in my enrolled chemistry lab activities at home, on campus, or elsewhere, there is a level of risk involved, and I need to have self-awareness regarding the risks associated with the lab activities.
- I understand that content in the “At-Home Laboratory Kit” is not suitable for children.
- I will be accountable for protecting myself, pets, and other individuals in the living environment.
- I understand that I am to seek professional medical advice for any medical concerns associated with my participation in this course.
- I understand that I am to seek professional medical advice for any medical concerns associated with my participation in this course. Student Health Services is available at (909) 274-4400.
- I understand that by agreeing to continue with the skill/lab requirements for my enrolled chemistry lab at Mt. San Antonio College, I should minimize risks by following all directions and safety protocols in the lab manual.
I agree to follow all applicable safety procedures to prevent harm by the following methods:
- Read the laboratory manual/handout or any other material that details experimental processes before conducting any hands-on activity, and contact the professor with any questions before proceeding.
- Read all warning signs and labels on all provided materials before engaging in any hands-on laboratory activities.
- Secure the “At-Home Laboratory Kit” in a way that protects children/pets/individuals from accidental injury or ingestion.
- The molecular model kit contains small parts, which are choking hazards for children below twelve (12) years old.
- Never eat/drink while handling the “At-Home Laboratory Kit.”
- After handling any material, wash hands thoroughly with soap and water, and wipe the work surface clean.
I understand that although the lab activities are generally safe, I am aware that I have the option to suspend my participation in a specific portion of a lab activity that I feel unsafe performing. If I am uncomfortable performing an activity, I agree to notify my instructor prior to the week that lab will be performed. I understand that I will still need to comprehend, learn about that lab content, and complete the lab report pages even if I do not perform the hands-on portion myself.
I will return the “At-Home Laboratory Kit” to campus at the designated scheduled drop-off date. I will verify that all items are included and comment on any lost or damaged items in the included “checklist.” If I withdraw from this course, I understand that I am still required to return the kit. I will keep all-original kit packages, containers, boxes, and bags unless instructed to dispose of after use. Failure to return the “At-Home Laboratory Kit” may result in a student misconduct report.
