Request for Active Military Benefits	Submit every semester to request certification of educational benefits.
Statement of Obligations Form	Submit one-time upon entering Mt. SAC.
Add/Drop/Withdrawal Form	Submit after your Request for Benefits if you add, drop, or withdraw from a course.
Parent Letter Request Form	Submit when requesting certification for a course you are taking at another school.
Submit Your DD-214 (Veterans Only)	Submit one time to verify your veteran status and military service. NOTE: Attach a PDF of ONLY your Member-4 or Service-2 copy.
Submit Your CalVet Fee Waiver Authorization	Submit each academic year after receiving your approved CalVet Fee Waiver authorization letter.
Submit Proof of VA Approval for Educational Benefits (Chapters 30, 33, and 35 should submit COE; Chapter 31 should submit VA Form 28-1905 from their VOC Rehab counselor; Chapter 1606 should submit NOBE)	Submit when first using VA educational benefits at Mt. SAC or when your eligibility information changes. For most students this proof will be a Certificate of Eligibility.
