Apply to ACCESS

Fill out an Application for Services online through the MyACCESS web portal.  and provide documentation of your condition.  

Documentation may include:

• Medical Verification of Disability - If you don't have documentation in written form, you may take the verification of your disability form to a licensed professional
• Psychological Verification of Disability -  If you don't have documentation in written form, you may take the verification of your disability form to a licensed professional
• Audiogram (hearing loss verification)
• Most recent Psycho Educational Assessment Report (may include IEP or 504 Plan but not required).
• Visual Acuity

Students are advised to register for classes on their registration date.
