How to Submit Immunization Records

    • 1Step 1 - Download Immunization Checklist
    • 2Step 2 - Complete the Immunization Checklist

Complete the Immunization Checklist by getting your 2-doses of rabies vaccine.

If you have health insurance, students with their own healthcare provider have been successful getting vaccinated through their own doctor by supplying a letter from the College. Click HERE for the letter!

If you do not have your own healthcare provider or you are not able to go through your healthcare provider, rabies immunizations are offered through companies such as The Healthy Traveler, Walgreens, GoodRx, Costco, CVS, and others.

    • 3Step 3 - Submit the Immunization Checklist

      Submit the completed Immunization Checklist
Please submit and upload your Immunization Checklist AND a copy of your immunization record to our Immunization Smartsheet.

Note: If you do not complete the Checklist and show proof of BOTH RABIES VACCINES, your submission will be discarded and may delay processing your clearance.

 Frequently Asked Questions

    • When should I submit my immunization?
As soon as possible! You can upload your immunization records when you have completed ALL 3 STEPS outlined above. To avoid delays in your registration for classes that require immunization, please submit your checklist and proof of both vaccines (or titer) as soon as possible. We recommend at least one full semester prior to the start of the class. This will ensure that you are eligible to register for the class once you pass the pre-requisite.  Your goal should be to become eligible to register before your priority registration date.
    • How much will it cost to get the immunizations if I do not currently have them?

Prices will vary from provider to provider. Some personal health insurance carriers can provide the vaccine upon request. You can also call one of the companies listed in Step 2 to inquire about the cost and receive it from them.

    • My personal health care provider is asking for documentation from the College about mandatory immunizations. Can I get a letter?
Yes, of course! A letter from the Vice President of Instruction can be found HERE.
    • Can I refuse to get immunization?

Yes, of course! We respect your privacy and we cannot force you to disclose any information you do not wish to. However, due to the new health regulation, the College is required to ensure that anyone who comes into contact or interacts with the animals through an Agricultural Sciences program or degree will need to provide proof of immunization. Therefore, you will not be eligible to register for the following classes since it would compromise the College’s and AVMA's legal mandate:

      • ASCI 51 - Handling & Restraint
      • AGHE 60 - Medical Nursing
      • AGHE 61 - Surgical Nursing
      • AGHE 65 - Radiography
      • AGHE 84B - Applied Animal Health Procedures
      • AGHE 83A - Work Experience
    • My medical records are confidential. Can I refuse to provide proof of immunization?

    • What if I have a medical condition that prevents me from getting some of the immunization? Can I be exempt from the immunization requirement?

Unfortunately not! While BP 5210 & AP 5210 provides some exemptions for employees and volunteers, the Mt. SAC Agricultural Programs have adopted strict health requirements mandated by the AVMA to ensure safety and well-being of the students who come into contact and interact with animals throughout the Programs.
