To access services offered by the CalWORKs, please review eligibility criteria on the Instructions form. If you qualify, fill out and submit all 5 forms listed in the CalWORKs form folder. Please note that ALL forms must be submitted in order for a program specialist to review. No incomplete packets will be reviewed.

How to Fill Out Forms Properly
- All students must have an A number before filling out forms. If you do not have an A number, please contact your CalWORKs Specialist.
- Login using your portal user name and password.
- Complete and submit all six forms.
- Must complete ALL "required" questions.
- Must click "submit" button after filling out each form.
