COVID-19 Vaccine Information

Los Angeles County Public Health has provided limited access codes for COVID-19 vaccines at county run sites.  Mt. SAC is making these available to select employees who are likely to be on the physical campus. If you are interested in receiving an access code, fill out your information below. Employees are encouraged to learn more about the COVID-19 vaccines by visiting the California Public Health COVID-19 Vaccines website and the U.S. Food and Drug Administration COVID-19 vaccines website. 

NOTE: At this time, access codes are limited to those who are working on the college campus AND names will be verified before codes are provided.
