Cleared4 Pass Information

For the latest COVID-19, vaccination, testing, or masking information, see www.mtsac.edu/health.

Students taking any classes and labs on-campus classes must either submit their COVID-19 vaccination record OR take weekly COVID-19 tests, free of charge, at convenient sites on campus.

To protect your privacy and personal info, you will not be required to display vaccine cards or test results. Mt. SAC is using Cleared4 to display a simple "cleared" or "not cleared" for Students, faculty, and staff who are on campus. All students who have classes on-campus were sent an email:

 SUBJECT: Welcome to Cleared4 at Mt. San Antonio College
In that email is your Cleared4 profile, with a website address like:
  https://www.c4wrk.com/1aBCDe2FGhijk3Lm4 
Please make a bookmark of your Cleared4 profile so you can update your vaccine or testing information, and to display your QR code when asked.

Questions about Testing?

Who can I speak to if I have questions regarding the College’s testing policy, or the testing process, such as the lab and testing materials?

• Student should contact Student Services Hot line at 909-274-5122 or VaxOrTest@mtsac.edu
• Staff/Faculty should contact their managers and/or Dean.

I am a minor, how does my parent or guardian provide consent for me to test?

• Please have your parent or guardian complete the minor consent form, and bring with you to the College testing site at the time of testing.
  Parents or guardians are NOT required to come to campus in person to sign the consent forms.

I have tested positive for COVID at another location (not on campus), how do I provide the results to the College?

• Please contact Mt. SAC’s contact tracing team at 909-274-6900 or ContactTracing@mtsac.edu, or by visiting https://www.mtsac.edu/health/ 

Accessing your Cleared4 Profile

If you do not know your Cleared4 account, you can try to access it or create a new profile by visiting  https://bit.ly/Cleared4 

Enter your A# or your username (the part before the @ sign of your Mt. SAC email address), and you will be sent an email or text message with your Cleared4 profile link.
