AS Funding
Request for Funding:
Use this form to request funding from the Associated Students. The form must be filled out completely and submitted to the Student Life Office a minimum of 6 weeks prior to the date of the event.  Your request will be voted on at a Senate meeting, held every Tuesday at 3:00pm.  Agenda items are due the Thursday before by 12:00pm.  A representative for your request MUST attend this meeting.
Banner Object Codes:
Advisors should use these codes when processing purchase requests through Banner. This spreadsheet lists the categories of items that can be purchased, as defined by Purchasing.
A.S. Annual Funding Request: 
Available January 23, 2014, To request funding from A.S. for the upcoming academic year, email the Director of Student Life at mtolano@mtsac.edu. These requests will be reviewed during the budgeting process, which concludes at the end of the spring semester. Submit completed A.S. Budget Request form to the A.S. Secretary in the Student Life Office (Building 9C-1) Due: February 20, 2014.
Vendor Set-up Packet
If you are doing business with a company (or vendor) that Mt. SAC has never worked with before, the new vendor must complete the attached form and submit it to Fiscal Services.
 
Applications
Conference Application:
Use this form to apply for any AS sponsored conference. All AS sponsored conferences are free to Mt. SAC students who have paid the current Activities Fee ($11 per semester)

Senate application:
Use these forms to apply for senate.  Job Descriptions  are found in the Organizational Directive.

Due September 12, 2014 at 10:00am to the Student Life Office; no exceptions will be made.

College Committee Application,  & Descriptions:
Use this form to apply to serve on a college committee as a student representative.

Student Court application:
Use these forms to apply for student court positions.  Positions will remain open until filled.
 
Elections
Student Trustee application:
Use this form to apply for the student trustee position. 
Due by Friday, March 20, 2015 by 10am to the Student Life Office; no exceptions will be made.
Executive Board application:
Use this form to apply for an executive board position. 
Due by Friday, March 20, 2015 by 10am to the Student Life Office; no exceptions will be made.
Elections Complaint Form: 
Use this form to file a formal complaint during an A.S. Election.
 
AS Documents
Appropriation Template:
(FOR SENATOR USE ONLY)
 - This form is only valid if it is written and co-authored by A.S. senators. Use this form to write an appropriation and request funding from the senate. Due by Thursday at noon the week prior to the meeting where it will be discussed.
Resolution Template: 
(FOR SENATOR USE ONLY) - Use this form to write a resolution. This form is only valid if it is written and co-authored by A.S. senators.
Bill Template: 
(FOR SENATE USE ONLY) - Use this form to write a Bill and request that senate adopt a new piece of legislation. This form is only valid if it is written and co-authored by a member of the Associated Students Senate. All other submissions will not be considered.
Recognition Template: 
(FOR SENATOR USE ONLY) - Use this form to write a recognition in order to officially recognize a people or events. This form is only valid if it is written and co-authored by A.S. senators.
Constitution: 
The A.S. Constitution is supreme governing document of the Associated Students.
ICC Constitution:
Read The ICC Constitution is the governing document of the Inter Club Council.
Organizational Directive:
Read this document if you'd like to know more about how Associated Students is structured, including job descriptions.
Publicity Directive: 
Read this document if you'd like to know how and where your organization can publicize on campus.
Elections Directive:
Read this document if you'd like to know more about Associated Students elections, how they are conducted and how you can run for office.
Judicial Directive:
Read this document if you'd like to know more about Student Court, how it operates, and its structure.
Financial Directive:

Committee Attendance Tracking Form
Use this form to track your attendance to college committee meetings.
Electronic Marquee Request
Complete this form in order to have a message displayed on the marquee on the Northwest corner of building 9C. Requests must be submitted at least 10 school days before an event.
Travel Release (Student or Staff
Complete this form before attending any College related event that takes place off-campus (e.g. conferences, retreats, field trips, etc.)
